*You will need to upload a Transcript/Report Card, Birth Certificate, Updated Shot Record, Parent/Guardian ID, Behavioral Report, and an IEP/504 for your scholar, if applicable, prior to enrollment processing.
You may also complete an application in-person during school hours once you have retrieved all of the required documents.
